Another part of the medical interview
is what is called the past medical history or PMH. And so usually we talk to patients about this
because what we want to do is we want to find
out if anything in the patient's past medical
history has an effect or is connected in any way to their current concern that they come to the
physician to see. Miss Smith, what I'd like
to talk to you about now is what we call your
past medical history. And the reason I talked to you about this is I want
to see if there's any connection to what brings you to the clinic today for. So that's what I'm
going to ask you some questions about
now. Is that right? Okay. All right. What I want to know about
is your adult illnesses. Conditions that as an adult
you've been diagnosed with. None so far. None so far. Any
conditions that you routinely see a physician
for or take medication for? No. No. Okay. Let's go back in time now now, your
childhood illnesses. Any childhood illnesses that you were treated for as a child? I think earaches. Okay. Is this
earache now that you have what you remember
having as a child? No, not really. I don't remember that much about them. Okay. You just know that
you were treated for them. Do you take medicines
do you take now? I don't take any
routine medicines. Any over-the-counter
medicines that you take? I'd say Tylenol on
occasion, that's it. Okay. Supplements or complementary alternative
medicines that you take? I take a multivitamin. All right. And you
take that every day? Yes. Okay. Past surgeries. Either as an outpatient
or an inpatient? No. Never have had surgery.
Any injuries that you've had? When I was a child,
I broke my arm. Okay. And did you see care at a doctor or a hospital for
treatment for that? Yes. And which arm was it? It was my right arm. Do you remember
how they treated it? I had to wear a cast. A cast. And have you had any
problems with it since then? No. Oh, okay. Ever been in
the hospital for any reason? No. What immunizations do you recall having or do
you remember having? Yeah, I think I'm up
to date on everything because I had my techni
shot at my last visit. All right. I've asked you about medications that you take. Do you have any allergies
to any medications? None that I know of. None that you
